Update Folder Malfunction - Help

When I change the source file during my workflow and then update the folder in EM2, EM2 brings all the photos from the folder in and not just the new additions.  This creates a redundancy of my photos (e.g. Source file contains 1000 photos and EM2 shows the 1000 photos.  When I add one and update in EM2, it brings in all 1001 photos and not just the new photo). 

Also, when I open the catalogue, the same source file shows up more than once and the images are sometimes split between the multiples of the same source file (e.g. The source file contains 1000 photos, but shows twice in EM2, once with 750 photos and again with 250 photos).

I cannot catalogue with these problems continuing.
Do you have any solutions?
